• SCHOOL'S DUTY OF CARE： The staff and the Management Committee of the Canberra Japanese Supplementary School Inc. (Below is the School) will take all reasonable care while students are in their charge, to protect them from injury and to supervise and guide their behaviour and activities.
• LIMIT OF DUTY OF CARE： Parents should be aware that the School is not responsible for injuries or damage to property that may occur during regular school hours, at any extra-School events (gyoji), or at the holiday school programme (junkai shido), except in the case of negligence. (Negligence is determined by due legal process associated with an insurance compensation claim application.)
• PARENTS' DUTY OF CARE： Parents should warn children of the risk to themselves, to others and to property, of impulsive, wilful or disobedient behaviour. Parents are also recommended to take out personal accident insurance and ambulance cover for their child.

AUTHORIZATION FOR ACTION IN CASE OF EMERGENCY

• I/we authorise for the School to make arrangements for the welfare of the student (including medical treatment), in an emergency.
• I/we understand that the School will attempt to make contact with the parents at the earliest opportunity.
• I/we agree to meet the costs associated with any emergency arrangements made by the School. Ambulance transportation is not free of charge in the ACT.
• Is there any medical condition that may affect your child at school, or in School-related activities? [Yes / No] If yes please give details and indicate treatment measures.

In case of severe allergies, or anaphylaxis, you are required to supply a separate Medical Emergency Action Plan to the school principal and your child's supervising teacher.

• 学校の注意義務：キャンベラ補習授業校の教師と運営委員（以下学校）は、児童生徒を事故から守り、その行動や活動を指導するため、児童生徒が学校の管理下にある間、常に相当な注意を払わねばならない。
• 注意義務の範囲：通常の授業日、行事、休暇中の特別プログラム（巡回指導）などにおいて外傷や器物破損が生じた場合、注意義務違反による明らかな過失によるものでない限り、学校には責任がないことを児童生徒の保護者は了解する。（過失が成立するかどうかは、損害賠償に関する訴訟手続きの中で決定される。）
• 保護者の注意義務：保護者は、衝動的、反抗的な行動や故意によって自身、他人、器物に及ぶ可能性のあるリスクに注意するように児童生徒に話しておかねばならない。保護者は、個人的に傷害保険や救急車の費用をカバーする保険に加入することを検討すべきである。

緊急時の対応に関する委任状

• 下記の保護者は、緊急時における児童生徒の安全確保のための手配（医療処置の手配を含む）を学校に委任します。
• 学校は、可能な限り早急に保護者に連絡をすることを試みる旨、了解しました。
• 下記保護者は、学校が手配した緊急時の対応に要した費用を支払うことに同意します。（ACTでは、救急車による病院への搬送は有料です。）
• お子さんには、学校での授業や関連活動に影響する何らかの症状がありますか？ （はい・いいえ）はいと答えた方は、下記に症状の詳細、対処方法を書いてください。

アレルギーやショック症状がある場合は、別紙に緊急対処手順を書き、校長叉は担任に提出してください。

 

下記の者は、学校の注意義務、注意義務の範囲、保護者の注意義務に関する上記の事項を読み、了解したことをここに認めます。また、緊急時の対応に関する上記の委任状に署名します。
